Club Activity Permit Form

Event application packets must be completed 45 days prior to the date of use. Your complete event packet must include:

  • San Mateo County Community College District Application and Permit for Use of Facilities
  • Club Activity Permit
  • Diagram of the set up for your event
  • Certificate of Liability Insurance (if applicable)
  • SMCCCD Food Service Provider Authorization Form (if applicable)
